Since The Wake I've been using Reaper, everything previous to that was recorded in Cakewalk Sonar.
Focusrite Scarlett 18i8 (3rd Gen) - My audio interface - simple, clean, cheap, reliable. I used a Audiofire Pre8 until it died in 2020ish.
(2) AKG C-414 mics - These are my workhorses. Most vocals and guitars are done through these.
(2) Neumann KM-184 mics - Beautiful mics for drum overheads, and acoustic guitars when I need a brighter sound.
(2) Shure SM-57 mics - I use these for toms and snare.
(1) Sennheiser MD-421 II mic - Used for my low tom.
(1) AKG D112 mic - This guy lives in my kick drum.
Universal Audio LA-610 - (2014-present) Fat, fat, fat. This is my workhorse that processes most of my signals these days. I don't usually use the EQ or compressor.
Sennheiser HD 598 Headphones - Super comfy open-back cans for mixing.
My favorite plugins:
FabFilter Pro-Q - The only EQ plugin you'll ever need. Incredibly quick and easy to get the results you want, with a great interface.
FabFilter Pro-C - Same for compression.
Valhalla Reverbs - Vintage and Plate are especially good.
SoundToys Echoboy - Amazing for subtle (or not so subtle) vocal thickening. I used to use Waves Kramer Master Tape for this, and still do at times.
Waves RVox - I've mostly de-Waves'd my setup, but this is so good for vocal compression that I still use it almost always.
FabFilter Pro-L2 - 99% of the time, I just slap this on my master bus and call it mastered.
FabFilter Saturn 2 - Saturation plugin that's amazing at warming and fuzzing sounds up.
Amplitube 5 - My go-to amp emulator. I almost never mic amps these days. American Tube Vintage is so good.
Guitar Rig 6 - My backup amp emulator when I can't find something on Amplitube.
Neural DSP Darkglass Ultra - Great bass amp emulator, especially when I need some dirt.
The Grandeur (Kontakt) - The general-use piano I reach for most often.
Addictive Drums - I almost always rough out my drums with this, and it's sometimes good enough to just keep.
Omnisphere and Keyscape - I reach for this first when I need sampled-y synth stuff. Absolutely massive collection of interesting sounds.

On my first CDs, acoustic guitar was usually recorded in stereo, usually with mics in XY configuration, panned 50-75%. These days I usually do mono, though for sparse mixes, I'll throw my KM-184s up in stereo. I'll always give it a high-pass, sometimes brutally if there's a bass instrument going. It usually gets some subtle compression to tame out the transients.
Vocals are usually mic'd about a foot back, with a pop screen near my mouth. My chain is currently Melodyne for pitch correction » Pro-G gate » Saturn 2 for warming it up » Pro-DS to slash the sibilance » Soothe 2 if I need any frequency mangling » Pro-Q to high-pass and bump 1k a bit » RVox for compression » EchoBoy and/or reverb.
Drums are usually the snare mic (SM-57 above and below), the bass drum mic (D-112), toms (SM57s and MD-421), two overheads in X/Y (KM184s), and a room mic (414 or whatever I have laying around). Most of my albums were recorded with my PDP kit, but in 2016 I replaced it with a Tama Starclassic Maple kit.
My dad built (!) most of my acoustic guitars. Two of them I use most often are similar but one is a shallower body, which has less boomy bass. I also have a classical guitar, an old Yamaha that's Nashville-strung, and as of 2022 a broken black piece of crap I bought for $150 that's weirdly fun to play.
Electric guitars are usually run direct in, and then through Amplitube or Guitar Rig. All electric on "Only Whispering" and before was with a Fender Fat Strat, then I used a lot of Telecaster, then around "Ashes" I added a Les Paul to the mix. Since 2016, I've pared down to the Les Paul and a new American Standard Telecaster. The amp mic is a MD-421 (Only Whispering mostly), a 414 (Dirty Wings/Not Quite Connected/The Simple Life) or the Apex ribbon mic (The Simple Life).
Bass guitars are direct in, sometimes through an amp emulator. Around "Only Whispering", I went from a crappy cheap generic to a Fender Deluxe Jazz active bass. In 2016, I added a Lakland 44-64 P-Bass clone with flatwound strings that I use for almost everything.
Piano and orchestra instruments are done using Soundfonts (early CDs), The Grand (middle-early CDs), Akoustik Piano / others (middle-later CDs), and NI Grandeur (later CDs).

"Here Today" was recorded with a standalone VS-1680 recorder, using MIDI drums, and internal guitar amp simulation.
"Crawford Street" is mostly done with a computer rig running Sonar 3 and 4. The interface was a mega-crappy Roland VM-3100 mixer. Electric guitars were mostly done through Amplitube.
"Only Whispering" was started with the VM-3100, but most of the parts were re-recorded using a Tascam FW-1840 interface. Electric guitars were mostly done through Amplitube.
"Dirty Wings", "Not Quite Connected" and "The Simple Life" were recorded with my new Onyx 400F, which I'm very happy with. The Telecaster was my main electric guitar, usually through the Tube Screamer into the Blackface, recoreded with a low volume into a 414.
"Breadcrumbs" is still with the Onyx 400F interface, using Sonar 8. I'm back to using mostly guitar amp emulators. I've also discovered the joys of Melodyne. The last three songs on the CD were recorded mostly with my new Portico 5012-H preamp.
"Ashes" was the last CD using Sonar. I'm using Reaper going forward. I also got an Audiofire Pre8 interface, since my Onyx finally crapped out.
